The Department of Personnel & Administrative Reforms stated that in pursuance of the implementation of the Chief Minister Health Insurance Scheme (CMHIS) which is to be operationalized from 1st October 10 2022 and as per the meeting held on 7th July 2022 in the Health & Family Welfare Department, all the regular Government Employees under the State Government have been directed to submit the details of the employees and dependent members of the family as per the prescribed format enclosed in hard and soft (in Excel format) latest by 5th September 2022.
An advance copy may be submitted through P&AR Department vide email i.e. CMHISPAR@gmail.com.
The criteria for dependency are as follows:
a) Natural parents
b) Spouse
c) Son: till he starts earning or attain age of 25 years or get married whichever is earlier.
d) Daughter /sister: till she starts earning or get married irrespective of age limit whichever is earlier.
e) Minor brothers: siblings below the age of 21 years.
f) Sons suffering from permanent disability of any kind both physical/mentally irrespective of age limit.
